Attentive Psychiatry (as data controller) is registered with the ICO registration number ZA536012 and conforms to GDPR regulations of storage and processing personal information. This privacy notice explains how the company uses any personal information it collects about you, as a service user.
Attentive Psychiatry provides psychiatry assessments and treatment and is based at 2A Prebend St, Islington, London N1 8PT
1. Why do I collect information about you
Information is stored about you as client of attentive psychiatry. The lawful reasons that your information is stored is under 'legitimate interests'. Broadly speaking legitimate Interests means that your personal information can be stored if there is a genuine and legitimate reason and your rights and interests are not being harmed. The other lawful reason by which your information is stored is under contract whereby services are sought from a health care professional. Under the contract of enquiring about health services from Attentive Psychiatry you agree to holding of personal information which will be kept secure under GDPR recommendations.
2. Why is your information stored?
Data is stored under legitimate reasons so that there is a clinical record of your treatment which is required under General Medical Council (GMC) guidance. Upon engaging with Attentive Psychiatry services all personal and sensitive information will be kept under password protected files.
Personal information will only be used for the purposes of fulfilling professional obligations for healthcare services under contract by you.
Any information shared by the client will be kept confidential with the exception of a letter written to the client’s GP which will be first shared with the client for accuracy.
With your agreement GPs may be contacted for background information by Attentive Psychiatry and similarly this information will be stored with appropriate security.
Confidentiality will otherwise be ensured unless discussed prior, for example, in correspondence with another professional involved in your care or an onward referral or unless it is in the vital interest of the public or the safety of the client.
3 How is your data stored:
Attentive Psychiatry will protect your privacy and the security of your data by using only encrypted and password protected means for data storage and methods of communication that are compliant with the General Data Protection Regulation (GDPR, 2018). Where service providers are based outside of the EU, there are assurances that they also comply with the standards of the GDPR.
Attentive Psychiatry Ltd will only store personal and sensitive information which is relevant to your treatment and/or assessment. It will protect your privacy and the security of your data by using only encrypted servers for data storage and methods of communication that are compliant with the General Data Protection Regulation (GDPR, 2018) such as 2 factor authentication.
4. How long is your information kept for - data retention
Under BMA and GMC guidance the storage of medical information is recommended for up to 20 years under the data protection act of 1998.
5. What information will be stored?
You may give the Practice personal information by filling in our Registration Form or by speaking with us over the telephone, emailing us or otherwise corresponding with us. The personal information you give us may include basic details such as your name, address, e-mail address, phone number, date of birth, next-of-kin contacts, GP information and medical insurance information for example.
We will also collect information relating to your health and medical history as well as details about the treatment you receive (special category information). The information you provide may be recorded in writing as part of the history taking.
We will also collect and process personal information that you and other medical professionals may send to us, such as referral letters, reports or assessments, or results of blood tests for example. We may also collect personal information about you that is given to us by your family members or other individuals known to you with your agreement.
6. How your personal information may be used:
Other clinicians involved in your care, including your GP or other referring professional, pharmacists, therapists or other medical professionals may be contacted with your details for their follow-up or advice.
We will seek your consent before sharing your personal information with such third parties.
If you do not consent to sharing of information it is important to discuss this at the assessment.
7. How to securely correspond with me
In order to reduce the risk of a data breach, it is requested that you password protect emails and to send password via text message. If you would like to send me private information it is recommended to password protecting it first and informing me of the password by phone or face-to-face.

To withdraw consent to me using your personal information in the ways outlined in this document.
You have rights relating to the information I hold to verify the accuracy or to ask for them to be supplemented, deleted, updated or corrected. You have the right to request a copy of the information that I hold about you. If you would like a copy of some or all of your personal information, please email or write to me via the contact details stated in this agreement. Information will be provided to you within 30 days.
I want to make sure that your information is accurate and up to date. You may ask me to correct or remove information you think is inaccurate. You have a right to request the transfer of your data to another individual or company.
10. Complaints or queries
I do my very best to meet the highest standards when collecting and using personal information. For this reason, I take any complaints I receive about this very seriously. I encourage people to bring it to my attention if they think that my collection or use of information is unfair, misleading or inappropriate.
